The Role

The Hospice Medical Social Worker (MSW) is a dedicated professional who, in accordance with the treatment plan, supports the physician and other team members by addressing the social and emotional factors related to health challenges.

Key Responsibilities :

• Provide social casework to individuals and families receiving nursing or related services from the Agency.
• Assess families' financial situations as appropriate, considering the patient's prognosis and medical needs, while facilitating referrals to financial assistance as needed.
• Respond to referrals for casework from Agency staff or outside professionals, delivering services as suitable.
• Participate in interdisciplinary team conferences to identify and discuss social issues, their impact on medical situations, and consider alternate solutions.
• Refer patients and families to community agencies, ensuring appropriate follow-up.
• Engage in case conferences with other agencies to coordinate care.
• Educate staff about social resources and health services available through special agencies.
• Ensure comprehension and application of the 4Ms (What Matters, Medications, Mentation, Mobility) while delivering Age-Friendly Care.
