Job Title : RN Case Manager

Location : Everett, Washington, 98201

Contract duration : 13 weeks

Shift : 5X8 -hours

Schedule : Days

Key Responsibilities :

• Coordinate and manage patient care across multiple departments, ensuring a smooth continuum of care.
• Conduct patient assessments, discharge planning, and care coordination activities.
• Collaborate with physicians, nursing staff, and social services to develop individualized care plans.
• Ensure compliance with CMS regulations and documentation requirements.
• Participate in interdisciplinary rounds and facilitate communication between healthcare team members.
• Float as needed to various units such as ED, ICU, Pediatrics, and specialty units
