Job Timings :   Regular business hours

Pay : USD 25 / hr - USD27 / hr on W2

Position summary :

This position supports the delivery of culturally competent, family-centered social work and care coordination for complex early intervention cases. As part of a transdisciplinary team, it involves working in natural environments—home, school, or community—and collaborating with public and private agencies. Key duties include care coordination, intake and assessment, family support, and consultation.

Key Responsibilities :

• Collaboration is frequent with a variety of multidisciplinary teams in public and private agencies, as well as community groups.
• This position is a member of a transdisciplinary team—speech-language pathologist, occupational therapist, physical therapist, special education teacher, etc.—and performs work in the child's natural environments such as the home, school, or community.
• Care coordination : 40%
• Intake and assessment : 30%
• Family support services : 20%
• Consultation : 5%
• Administrative and other duties : 5%
