RN Nurse Care Manager

A company is looking for a Nurse Care Manager for the Eastern Time Zone.

Key Responsibilities

Manage the overall care management of an assigned patient panel, leveraging nursing expertise and value-based care operations

Establish trusting relationships with patients and families to provide clinical and emotional support

Coordinate interdisciplinary approaches to achieve continuity of care and monitor the effectiveness of care management plans

Required Qualifications

Associate degree in nursing

Current RN License is required; Compact License preferred

Care management experience is required

Certified Case Manager preferred

Intermediate skills with MS Office Suite, including Outlook and Teams
