Job Summary

We are seeking a qualified Hospital-Based Social Worker to provide support in a behavioral health services (BHS) environment. This role plays a critical part in patient care coordination, discharge planning, and psychosocial assessments within a hospital setting. Candidates must be licensed (LCSW) in Georgia and have active BLS certification . Key Responsibilities

• Provide clinical social work services in a hospital-based behavioral health unit.
• Conduct psychosocial assessments and coordinate with interdisciplinary medical teams.
• Develop care plans and provide emotional support to patients and families .
• Support patient transitions and discharge planning with internal and external stakeholders.
• Maintain timely documentation aligned with hospital and regulatory standards.
