Job Summary
Medix is seeking dedicated Social Workers to be a part of a multidisciplinary team working within the CalAIM program. This role involves providing comprehensive support services to Medi-Cal beneficiaries experiencing homelessness. Social Workers will work alongside care managers, community health workers, and other specialists to address complex physical, behavioral, and developmental health needs to improve health outcomes and reduce the use of high-cost services.
Responsibilities / Job Duties
- Provide therapeutic supportive services to address mental health, functional, and substance use barriers.
- Conduct crisis and suicide risk assessments and provide appropriate interventions as needed.
- Offer crisis intervention services to enhance members' coping skills and problem-solving abilities.
- Support case management by conducting biopsychosocial assessments for new members.
- Follow up on assessments identifying mental health and safety concerns.
- Provide intentional case management support to team members to transition patients to more independent recovery services.
Qualifications
Education
Bachelor's Degree in Social Work, Psychology, or any behavioral health fieldQualifications
Able to maintain confidential information according to HIPAA standards.Experience in homeless services or related social services fields.Strong case conference skills and holistic approach in housing retention.Familiarity with homelessness, substance abuse, mental health diagnoses, and trauma.Training in Motivational Interviewing, Trauma Informed Care, and other related trainings preferred.Experience
One year of experience in homeless services, mental health, substance use, co-occurring disorders, or working with vulnerable populations.Experience in psychosocial assessments and field-based case management interventions.Skills
Proficient in Harm Reduction, Motivational Interviewing, and Crisis Intervention skills.High level of tolerance and empathy for individuals with urgent or multiple case management needs.Schedule / Shift
Monday-Friday 8-5pm ONSITE
