Family Practice / Primary Care Nurse Practitioner

Location : Remote, Cedar Rapids, IA (Candidates should live close to the targeted city and may occasionally be asked to visit patients' homes for specific situations, though this is rare.)

Required Certifications :

Board Certified

State licensure in IA

Previous Experience Desired :

2 years of direct patient care experience related to polychronic conditions

Start Date : ASAP

Duration / Schedule : 3 months with likely extensions

On-call / Callback : No call required

Types of Cases / Procedures :

Maintain quality of care standards

Assist patients with enrolling in educational programs

Knowledge of disease diagnosis and prevention

Assess patients' health status

Implement and follow up on care plans

Document patient information accurately and timely

Patient Volume : Complete 3+ patient visits daily

Additional Notes :

This role primarily involves virtual work with occasional travel within a specific geography for in-home assessments.

Training includes 3 days onsite and the remainder virtually.

Monday to Friday, 8am-5pm in the candidate's time zone.
