job_description.job_card.job_descriptionActive and unrestricted NP or PA license in the state of practice (or ability to obtain by start date) NP : Graduate of an accredited MSN or DNP program PA : Graduate of an accredited PA program Board certification : NP : Certified by AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute Care PA : Certified by NCCPA Current DEA license and prescriptive authority Ability to meet physical requirements (e.g., lifting 30 lbs., navigating stairs, kneeling for assessments) Reliable transportation Ability to obtain collaborative practice agreement if required by state Full COVID-19 vaccination (per federal, state, and client requirements) Drug screening required prior to employment 1+ year of clinical experience (long-term care preferred) Experience with complex behavioral, social, or functional patient needs Understanding of geriatrics, chronic illness, and end-of-life care Proficiency in electronic medical records and documentation
Advanced Practice Clinician
Location : Spokane, WA (Travel between multiple senior living facilities)
Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM
Travel Required : Yes – 100% travel between senior living facilities during shift
Position Summary
Seeking an Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk health plan members in senior living facilities. This role offers flexible scheduling with no on-call, weekend, or holiday requirements, and is part of an interdisciplinary team focused on improving outcomes for patients with complex medical and behavioral needs.
Primary Responsibilities
- Conduct comprehensive patient assessments
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate documentation, including ICD-10 coding
- Collaborate with interdisciplinary care teams
- Lead advanced illness and care planning conversations
- Provide education and counseling to patients and caregivers
