Location : IU Health Gateway Building, Indianapolis (Primarily Virtual with In-Person Training / Meetings)
Shift : Full-Time Days | 8 : 30 AM - 5 : 00 PM, Monday - Friday

Indiana University Health is seeking a dedicated Licensed Master Social Worker (LCSW) to provide high-quality, virtual psychosocial support across the IU Health System. This role involves conducting assessments, developing treatment plans, coordinating resources, and collaborating with healthcare teams to ensure safe, effective, and patient-centered care for diverse populations.

Position Overview :
The Integrated Social Work team provides virtual social work services to all primary care offices throughout the IU Health System. This role offers virtual urgent consultations to patients and their families, assisting with psychosocial assessments, treatment planning, and resource coordination. This role involves serving as a resource and role model for students, interns, LBSWs, and LSWs, while collaborating closely with the clinical team to develop individualized treatment plans that address psychosocial needs and promote safe, effective discharge and care continuity.
Key Responsibilities :
- Conduct face-to-face assessments and counseling with patients and families, primarily through virtual platforms
- Assist in developing educational materials for patients, families, and healthcare team members
- Serve as a resource and role model for students, interns, LBSWs, and other social work staff
- Collaborate with multidisciplinary teams to develop individualized treatment plans addressing psychosocial needs
- Facilitate safe and effective discharge planning and care transitions
- Interpret and communicate complex patient / family needs with compassion and professionalism
- Evaluate psychosocial risk factors, including prior functioning, social support, reactions to illness, and coping skills
- Counsel patients and families on emotional, social, and financial impacts of health conditions or disabilities
- Access and mobilize community and family resources to meet identified needs
- Serve as a liaison with child and adult protective services, as well as external community agencies, in cases of abuse, neglect, or domestic violence
- Maintain principles and techniques of crisis intervention and de-escalation, providing leadership during acute crises
- Provide intervention in guardianship, foster care, adoption, and mental health placements
- Offer specialized mental health services, including crisis intervention, addiction assessments, and psychiatric referral recommendations
Qualifications & Requirements :
Education : Master's Degree in Social Work from an accredited institution required.Licensure : Active, permanent Indiana license as an LCSW required.Certifications :Crisis Prevention Intervention or alternative de-escalation training (required within 6 months of hire).
May be scheduled for on-call and holiday rotations.Experience : 3-5 years relevant healthcare, mental health, or community-based experience preferred.Skills & Knowledge :Expertise in psychosocial assessments, therapeutic counseling, crisis intervention, and substance use disorder services.
Knowledge of disease processes, human behavior, and social determinants of health.Strong discharge planning skills for safe patient transitions.Familiarity with legal mandates for protective services (child and adult).Ability to manage dangerous or potentially dangerous situations using de-escalation techniques.Experience working with diverse populations across geographic, cultural, and financial backgrounds.Proficiency in documentation within electronic medical records and Microsoft Office Suite.Why Join IU Health?
