Role Scope :

We are looking for a dynamic supervisor who is passionate about supporting elderly persons in their journey. Under supervision of the Center Manager the Social Services Supervisor is responsible for supervising and coaching master-level social-workers as well as the New Participant Advocate. The Social Services Supervisor is part of the center leadership team to ensure compliance with assessments and care plans.

Core Responsibilities & Expectations for the Role

• Contribute to a center experience that Participants want to spend time in a team culture that cares and creates joy and an environment where all participants and team members belong.
• Continue to raise the bar. Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants.
• Exhibit and honor Habitats values.
• Coach MSW level social workers to collaborate as part of the Interdisciplinary Team (IDT) to optimize health status and quality of life of the participants as part of the care plan.
• Ensure compliance with social work assessments to determine psychosocial needs preferences and goals of the participants
• Monitor social work team documentation with interventions as agreed upon in the participants care plan including but not limited to arranging necessary resources and services assisting with care transitions providing individual as well as group counseling and case management.
• Monitor and coach MSW team collaboration with primary care providers and other members of the care team to guide smooth care transitions between settings (e.g. hospitals skilled nursing facilities home etc.)
• Role model how to Initiate coordinate and facilitate care conference meetings to ensure the highest level of care coordination among other care team members participants and other people within the participants support network (family informal caregivers etc.)
• Ensure discharge planning when participants disenroll from the program is thorough and timely
• Develop train and monitor the components of the new participant experience inclusive of new participant welcome process and liaison with Outreach & Enrollment team members
• Performs related duties as assigned.

We take into account an individuals qualifications skillset and experience in determining final salary. This role is eligible for medical / dental / vision insurance short and long-term disability life insurance flexible spending accounts 401(k) savings paid time off and company-paid holidays. The expected salary range for this position is $91000-$109000 annually and is a bonus-eligible position. The actual offer will be at the companys sole discretion and determined by relevant business considerations including the final candidates qualifications years of experience skillset and geographic location.
