POSITION SUMMARY

The Social Worker will be an integral part ofthe Hospice Interdisciplinary Team and will assist the team in understandingthe significant social, emotional and cultural factors related to each patient'sterminal illness. The Social Workerconducts psychosocial assessments and evaluations, as well as provide emotionalcounseling to patients / families in their place of residence - in person andtelephonically. The Social Worker worksunder the direction of a physician and supervision of the Director of PatientCare Services and serves a as resource to our patients, families and ourcommunity. Must be able to utilizecomputers and be comfortable with electronic medical records.
