DaVita is looking for a Licensed Social Worker (LMSW) to support our patients and their families at our Dialysis Clinics in the greater Cass Lake, MN area!

Here is what you can expect when you join our Village as a Social Worker (MSW) :

Schedule : Monday - Friday 8 AM - 5 PM

Full time status - 40 hours

Home clinic will be Cass Lake Dialysis Clinic with temporary coverage over Grand Forks and Fargo. Once temporary coverage ends you will still obtain 40 hours through other clinics in the area.

Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them.

Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

Requirements :

Licensed or registered in the state of practice as required by state regulations LMSW or LCSW

Meets all state required regulations to practice in the dialysis setting including LICSW or LGSW licensure.

Master's degree in Social Work (MSW) required with a specialization in clinical practice

Demonstrated knowledge of government and private insurance programs

Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

The Salary Range for the role is $59,000 - $95,000 per year.
