Position Summary

The Psychiatric Nurse Practitioner will provide psychiatric evaluations, medication management, and follow-up care to adult patients within our internal medicine framework. The ideal candidate will be passionate about integrated care, demonstrate clinical excellence, and be comfortable managing a range of psychiatric conditions in coordination with primary care providers.

Key Responsibilities

• Conduct comprehensive psychiatric assessments and develop individualized treatment plans
• Provide medication management and monitor patient progress and side effects
• Collaborate closely with internal medicine providers to ensure continuity of care
• Deliver care both in-person and via telemedicine platforms
• Maintain thorough and timely documentation in the electronic medical record (EMR)
• Educate patients and families on treatment options and care plans
• Participate in interdisciplinary collaboration and quality improvement initiatives
