Job ID : 134290 Direct Hire Physical Therapist Role $41.73-$54.28 per hour in Loveland, Colorado Clinical Magnet (Part of ICON Medical) is looking for a Physical Therapist to join our partner’s full-time team in the Loveland, Colorado area. This is a DIRECT HIRE OPPORTUNITY! Key Responsibilities for the Physical Therapist : Evaluate and treat program participants (older adult and geriatric) on acute, restorative, or maintenance levels of care. Develop and integrate physical therapy treatment plans into the Plan of Care (POC) as directed by physician orders. Collaborate with the interdisciplinary team, caregivers, and facility staff to promote safety, mobility, and independence. Provide patient and caregiver education on therapy plans, DME (Durable Medical Equipment), and home exercise programs. Supervise and delegate therapy interventions to PTAs and Rehab Aides in alignment with state regulations. Conduct home visits and educational sessions as needed, documenting all care in participant records. This will be 10% of your time. Key Requirements for the Physical Therapist : Bachelor’s Degree in Physical Therapy (Master’s or Doctorate preferred). Graduate of an accredited Physical Therapy program. Active PT license in the state of practice. 1 year of experience working with the frail or elderly population (required). Strong communication and collaboration skills, with experience working in interdisciplinary care teams. Compensation and Schedule for the Physical Therapist : $41.73–$54.28 / hr (compensation may vary depending on experience).
