Sobe Innovative Rehabilitation is a leading provider of in-home physical, occupational and speech therapy services, dedicated to delivering high-quality care to patients in their homes. We are seeking an experienced home health scheduler to coordinate in-home therapy visits efficiently, ensuring patients receive timely care while optimizing therapist schedules.

The Home Health Scheduler is responsible for scheduling and coordinating in-home therapy visits for physical therapists, occupational therapists, and speech therapists. This role requires strong organizational skills, attention to detail, and the ability to balance patient needs and therapist availability. Key Responsibilities

• Schedule and coordinate in-home (home health) therapy visits based on patient needs, therapist availability, and geographic location.
• Monitor visit frequencies to ensure compliance with physician orders, therapy plans of care, and insurance regulations.
• Communicate effectively with therapists, patients, families, and caregivers as needed and relay schedule changes.
• Manage cancellations, reschedules, and urgent visit requests while maintaining efficiency.
• Maintain accurate records in the EMR system, updating schedules in real-time and documenting any patient or therapist changes.
• Track patient progress and scheduled visit completion to ensure timely discharges and compliance with care plans.
