PURPOSE :

The Housing Case Manager provides individualized, comprehensive support to residents living in transitional or permanent housing. This includes facilitating life skills programs, collaborating with internal teams and external partners, and supporting long-term stability through proactive case management.

Requirements

Core Responsibilities

Conduct comprehensive client assessments and identify critical needs.

Collaborate with internal teams and external partners to coordinate services.

Maintain documentation according to agency policy and program requirements.

Provide crisis intervention and follow organizational safety protocols.

Uphold professional ethics, integrity, and accountability in all interactions.

Maintain confidentiality regarding client, agency, and staff information.

Advocate for clients in a fair, respectful, and culturally sensitive manner.

Case Planning & Documentation Compliance

Develop and maintain individualized, client-driven case plans with SMART goals, including short- and long-term objectives.

Regularly monitor progress and adjust plans as needed, empowering clients to access resources that support goal attainment and long-term stability.

Ensure all assessments (e.g., VISPDAT, SPDAT); logs, and reports are accurate, up-to-date, and compliant with agency, HUD, and funding requirements.

Document client interactions, interventions, and any critical incidents.

Report urgent incidents immediately to the Program or Regional Director.

Collect and provide data for program reporting, audits, and quality assurance.

Ensure all documentation adheres to professional, ethical, and confidentiality standards.

Housing-Specific Duties

Facilitate life skills and training programs (budgeting, tenancy, financial literacy, wellness, decision-making).

Conduct property inspections to ensure a safe and supportive environment.

Coordinate with internal teams and external partners to support client housing stability.

Provide supportive crisis intervention using trauma-informed and solution-focused approaches.

Professional Credentialing & Development

Maintain active certification through CEUs, training, and practicum hours.

Demonstrate commitment to ongoing professional development in Trauma-Informed Care, Motivational Interviewing, and Supportive Housing & Life Skills Approach.

Ensure all client care and documentation meets professional, ethical, and regulatory standards.

Qualifications

GED or High School Diploma required; Associate or Bachelor's degree in social work, Human Services, or related field preferred.

Experience with vulnerable populations (homeless, mental health, substance use, survivors of domestic violence, medically frail).

Strong interpersonal, problem-solving, and conflict resolution skills.

Ability to uphold confidentiality and professional boundaries.

Valid Utah driver's license and clean driving record (required for roles involving transportation).

Commitment to ethical practice and ability to navigate ethical dilemmas.

Preferred :

Trauma-Informed Care, Motivational Interviewing, and Supportive Housing & Life Skills Approach knowledge.

Experience with HUD, TFA, UHMIS, UHIN or medical case management documentation systems.

Must hold current Utah Case Management Certification (or program-specific equivalent) prior to hiring.

Physical Requirements & Work Environment

Must work on-site as required, interact with clients, and move around the facility.

Ability to lift 25 lbs. and perform duties in crisis or fast-paced situations.

COMPENSATION

Pay is $19 an hour.
