job_description.job_card.job_descriptionMedical, Dental, and Vision Insurance Life Insurance Disability Insurance 403b PTO Paid Holidays Flexible Spending Account Employee Assistance Program Conducts psychiatric assessments including psychiatric history, course of illness, response to treatment, mental status examination, and DSM-V / ICD-10 diagnoses assists the team in determining the best treatment approach, with input from the individual and the family. Provides medication management and oversees the medication self-administration program for all persons served by the FACT program to ensure symptom reduction and stability of persons served. Works closely with the FACT team and utilizes treatment strategies which include : assessing each persons behaviors and symptoms of mental illness and prescribing medication in response to the symptoms; regularly reviewing and documenting the persons symptoms of mental illness as well as his or her response to the prescribed medication treatment; educating the person regarding the symptoms of his / her illness and the effects and side effects of the medication prescribed to regulate it; assisting the person to identify the symptoms and occurrence patterns and to develop internal, behavioral, or adaptive methods to help lessen their effects; monitoring, treating, and documenting any medication side effects; developing and maintaining a positive, therapeutic relationship with the person served; using new atypical medications with greater effectiveness and a better side effect profile; and simplifying the medication regimen. Completes screenings, assessments, and / or regular visits in both office and field-based settings as identified on the individuals treatment plan. This may include visits to hospitals, CSUs, jails, or other community living environments. Attends and participates in the teams daily organizational meetings to assess patient status and progress, to coordinate treatment activities, and to develop treatment solutions to identified problems. Coordinates, with the nurses, the medical treatment of all individuals enrolled in the program and consults with both staff and other agencies regarding diagnostic, medical, and treatment problems of persons served. Provides accordance with State Regulations, MHRC Policies, Procedures and Standards, and the Program for Assertive Community Treatment (PACT) manual. In order to be considered, a candidate must have a Master's Degree in Nursing and current APRN Florida State Nursing license required. ANCC certification in Psychiatric Nursing required. DEA Certification required. One year experience in psychiatric nursing with working with individuals with severe and persistent mental illness required. Must demonstrate proficiency in the MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment. Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff and other department professionals. Each employee contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness.

Position Description
Mental Health Resource Center is looking for a Psychiatric Nurse Practitioner to join its Florida Assertive Community Treatment (FACT) Program that serves individuals in the Tampa, Florida area.
The FACT program serves adults with severe and persistent mental illness, who may also have a co-occurring substance abuse disorder. The programs multidisciplinary staff obtain or directly provide all mental health services to participants, primarily in their homes or the community, including : Psychiatric Care, Medical Referral and Follow-Up, Individual Supportive Therapy, Crisis Assessment and Intervention, Substance Abuse Services, Employment Support Services, Support in Activities of Daily Living, Social Network Support and Development, Peer Recovery Support, and Case Management Services.
A minimum of 75% of the FACT teams services are provided in the persons home or community environment. Company vehicles are available; however staff may also need to use their personal vehicles to provide services.
