Pride Health is hiring Home Health RNs to support our client in Hoover, AL 35244. This is a 3+Months assignment (Possibility of extension or conversion). opportunity and a great way to start working with a top-tier healthcare organization!

Position Summary :

Primary functions of the Registered Nurse are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient / family and referring agency and assume the responsibility for coordination of care.

Essential Duties and Responsibilities :

• Assess and monitor home health or hospice patients, developing and updating individualized Plans of Care based on physical, psychosocial, and environmental needs.
• Maintain accurate and timely documentation of patient care, ensuring quality, continuity, and compliance with regulatory and third-party payer requirements.
• Educate patients and families on medications, care techniques, safety, and self-care based on age, condition, and readiness to learn.
• Supervise and guide support staff (HHA / STNA / LVN) and coordinate referrals to other healthcare agencies as needed.
• Perform on-call duties, participate in interdisciplinary team meetings, and communicate effectively with all parties involved in patient care.

Expected hours : 40 hours in a week

Local Pay range : $40-$44 / hour

Travel Gross Pay : $2100 - $2200 / Week
