Position : Director of Social Services
Location : Lakeland, Florida
Shift : Full-Time
Salary : Up to $65,000 per year
Join Our Team as the Director of Social Services
Wedgewood Healthcare and Rehabilitation is seeking a compassionate and skilled Director of Social Services to lead our department in delivering exceptional, resident-centered care. This is an opportunity for a strong leader with clinical insight, regulatory knowledge, and a genuine passion for enhancing the lives of residents.
Key Responsibilities
Develop, implement, and maintain departmental policies and procedures in compliance with all regulatory requirements.
Recruit, mentor, and manage a dedicated team of social services professionals.
Uphold and advocate for the Resident Bill of Rights, fostering dignity and respect in every interaction.
Actively participate in interdisciplinary care planning meetings.
Lead discharge planning to ensure smooth, effective resident transitions.
Monitor and evaluate the quality of social services to continuously improve resident outcomes.
Collaborate with other departments to maintain compliance with documentation standards.
Prepare and present Quality Assurance (QA) reports as needed.
Serve as the facility’s Grievance Officer , addressing resident and family concerns with empathy, fairness, and professionalism.
Qualifications
Bachelor’s Degree in Social Work (BSW) or related human services field (required)
Minimum of 2 years of experience in a healthcare or medical setting
Strong knowledge of federal and state regulations related to long-term care social services
