Sadler Health Center is recruiting a Community Health Workers to join our team based out of our Carlisle or our West Shore Clinic!

We are looking for those who can establish, build and maintain trusting working relationships with patients and their families in order to assess and identify what appropriate programs they may need or are eligible for. In this role you will work closely with our team of care coordinators, patient enrollment specialists, pharmaceutical support team and case managers.

You will be expected to provide support to our patients and their families to establish primary care for health, dental, vision, behavioral health and assist patients and navigate systems to obtain resources or arrange for transportation to appointments when needed.

At times you will assist in communication between providers and patients and involved loved ones to ensure service needs are achieved.

You must be comfortable with maintaining communications by both phone interaction and face to face visits.

Travel within our service are of Perry and Cumberland county is required.

One position will be based out of Carlisle and the other out of our West Shore Clinic.

Qualifications :

• Works independently and is self-directed.
• High School Diploma or GED.
• Organizes, prioritizes, and coordinates multiple activities and tasks.
• Graduate from an accredited Medical / Dental assisting training program
• Proficiency in the use of Microsoft Office applications; Word, Excel and Outlook
• Customer Service experience
• Effectively communicates orally and in writing.
• Ability to effectively and professionally represent the organization.
• Ability to work in a team environment.
• Remains calm and effective in high pressure and emergency situations.
• Ability to organize data and medical records in a manner consistent with national standards.
• Demonstrates knowledge of medical terminology.
• Must be dependable and able to maintain punctual attendance and willingness to accommodate a flexible work schedule.

Preferred :

Pay for this role is $21 / hr

Must acquire Community Health Worker Certification within 12 months.
