This is a Full Time Day Shift position, typically from 8 : 00am-4 : 30pm Monday - Friday with rotating weekends.

The Case Manager Registered Nurse plays a crucial role by providing exceptional patient care and ensuring effective care transitions. Responsibilities include coordinating care plans, managing length of stay, and collaborating with multidisciplinary teams to ensure the highest quality of patient-centered care.

Main Responsibilities :

  • Conduct comprehensive assessments of patient and family needs, considering available resources.
  • Reassess clinical conditions and develop strategies to reduce readmission risks.
  • Collaborate with healthcare teams to coordinate care and discharge plans.
  • Ensure timely referrals to Social Services for psychosocial needs.
  • Facilitate effective communication among physicians, patients, and hospital staff.
  • Advocate for patient rights and ensure the alignment of care with patients' cultural and personal values.

    • What You Will Need :

  • Current Registered Nurse license in Texas required.
  • Associate Degree in Nursing or Nursing diploma required; Bachelor's Degree preferred.
  • 3+ years of clinical nursing experience or 2+ years in case management.
  • Certification in case management is preferred.
  • Experience with InterQual criteria is a plus.
