Description :
RN - Case Manager position available.
The facility has a capacity of 457 licensed beds and includes various specialized services.
The average census and case load is typically at 99% capacity, with 14-16 patients per day depending on the unit assigned.
Successful candidates must have a minimum of 3-5 years of recent experience in Acute Care Case Management.
First-time travelers will not be accepted; candidates must have previous travel experience as an RN Case Manager.
Patient population varies and includes Adult Medical / Surgical, Cardiac, Ortho, and possibly Pediatrics.
Required skills include experience in placement for skilled nursing facilities, inpatient rehabilitation, long-term acute care, hospice, home health, durable medical equipment, and behavioral health needs.
Ability to work with complex placements, including the homeless population, is essential.
The department works collaboratively with supportive staff, including a director, managers, and social workers.
Technology used within the department includes EMR systems and referral management tools.
Candidates should be adaptable to different units, and scheduling may include a weekend rotation.
Travelers will be expected to work all holidays unless approved for time off.
The department culture is supportive and teamwork-oriented.
Managers maintain open communication about changes, and educators provide support through transitions.
Must possess care management skills, including assessment and discharge planning.
